Terms And Conditions
In checking the box below I am voluntarily choosing to participate (virtually or in person) in an exercise program and/or use exercise equipment at Discover Strength Personal Fitness Center, Inc. ("Discover Strength"). I may use Discover Strength's facility, equipment and machines, and I may receive instruction from Discover Strength's employees during my workouts (virtually or in person). My participation in such activities involves an inherent risk of injury due to various circumstances such as unknown health concerns, equipment failure, or the actions or inactions of other Discover Strength customers, Discover Strength employees, and me.
I agree to assume all risk and responsibility surrounding my participation in exercise activities at/with Discover Strength. To the maximum extent permitted by law, I agree to release, hold harmless, and indemnify Discover Strength and its officers, directors, staff, representatives, employees, agents, all owners of property on which activities take place, and parties who provide goods or services in connection with Discover Strength activities, from and against all present or future claims, losses or liabilities for injuries which I may suffer, or for which I may be liable to any other person resulting from the above-mentioned activities, due to the acts or omissions of other Discover Strength customers, Discover Strength employees, me, or anyone. I agree that this Waiver and Release of Liability will apply and act as a complete defense to claims made by me or on my behalf, or by heirs or personal representatives, or assigns, each of which is bound by the terms of this Waiver and Release of Liability.
I HAVE READ THIS WAIVER AND RELEASE CAREFULLY AND, HAVING DONE SO, I AM SIGNING IT VOLUNTARILY.
